Completion of this paper will provide you the opportunity to explore data related to health status and health outcomes in a specific county of your choice. Understanding data is central to developing community/public health policies and interventions.

General Instructions:
1. Complete the Community Assessment Activity Sheet. This will guide you as you navigate through the variety of resources. Students should examine their county of interest for background data as well as population strengths and weaknesses.
2. Review the Community Assessment Instructions for the paper. Completion of this paper will provide you with the opportunity to explore data related to health status and health outcomes in a specific county of your choice. Understanding data is central to understanding levels of prevention and developing health policies.
3. Review the Community Assessment Paper rubric included in the instructions.
4. The paper should be a minimum of 3 pages and a maximum of 4 pages (does not include title or reference page)
